On 1/7/58, Commissioner WILLIAM PHILIPS, New York State Liquor Authority, made available to SA [REDACTED] the original of a NYSLA "Employee's Questionnaire" for subject. It is dated 5/27/36, and reflects subject's photograph, description and signature on the front and his fingerprints on the back.
The questionnaire was photographed at the NYO and the original returned to Commissioner PHILIPS, 1/8/58.
One copy of the questionnaire is being forwarded to Buffalo with three extra copies of the photograph.
One copy of the questionnaire is being forwarded to the Identification Division.
